Endorsement of A320/B737 on Indian Licence

It's been a while since Jet endorsement related queries were posted here. I thought this would clear up a lot of doubts.

What are the new fee for endorsement of Type and IR. Is it still Rs5000 each, so that we pay Rs 10,000 all up for Type endorsement and IR renewal.

Has there been any change in set of documents and paperwork to submit at DGCA office.

Where are the specific DGCA proformas for skill test/checks etc for Jet acft.

Has anyone faced problem from DGCA trying to get endorsement on their licence.
