r22 accident - report

this report below caught my interest. do you think the initial "wobbly" pickup had something to do with the later accident? seems a bit odd.

Seems like he was overloaded to me. . But no PAX weights. . ?

Overloaded, or could have caught fire from spilling fuel, could have been an engine failure, could have been turbulence and heavy a/c. There are too many likely possiilities.
That the initial take-off was observed as uncontrolled there could have been a strike of the MR blades which after 18 minutes of flight caused a mechanical failure.
The final report should clarify what went wrong.
