News Copter Twin Squirrel Crash USA - what was the cause?

Just seen the (old) video of the news chopper crashing in the States a couple of years back..what was the cause? Tried the FAA site but to no avail.

Well now. You'll have to give us something more to go on than "twin squirrel crash." Got an N-number? Rough date of occurence? Location (other than "U.S.")? Personally, I can't recall any Twinstar ENG crashes in the last couple of years. There was the recent single-Squirrel crash due to...uhh, well it was either a tail rotor failure or hydraulic failure, one or the other and the pilot didn't seem to know how to ascertain the difference... Then there was the EC-135 crash in the N.Y. area some years back caused by...uhh, well...you've got to read that one to believe it. Both were captured on video.

A news chopper4 ship went into the Passaic River in New Jersey in Dec, 1998. I believe there was video of this one , too.

Yep, that one. A colleague was asking about it. Was it Tail Rotor or Hyd failiure?

The NTSB report says nothing about initial failures of any of the helicopter-systems. To me it looks like they're spot on with their analysis of the EC-135 accident and that the failure seemed to be somewhere between ear no.1 and ear no.2 on the pilot.

But I could be wrong, it's happened before.
