Lessons Learned....At Too High A Cost!
Cut through the chaff on this one....wipe away the rolled shoulders and feigned lack of recall...reject the bovine fecal matter.....and see if your aviation unit has any similarity to this one.
It matters not which military service or civilian aviation operation....we all rely upon well thought out procedures, crew manning, and operational demands.
When we take shortcuts or patently ignore established effective procedures....do we set ourselves up for failure?
Aviation is an unforgiving business....and should be more so when we kill one of our own.

Remember too, it takes a strong (and safe) experienced mind to say NO to the chain of command.

The Supervisor ran it up the food chain but no action was taken....there is where I took issue with what happened.
When your subordinate, well trained, experienced, and concerned, comes to you with a problem and is not satisfied with your response and then goes higher yet....who is the problem?
Is it the subordinate or the superior?
The accident could have been prevented by a simple check of everyone being clear of the external stores immediately before pushing the "Jettison All Stores" Button....but that was not done.
After that....there are a lot of other factors, actions, and in-action that lead to this Death.
Taking Personnel off the work party to accomplish a drug test.....which is a standard procedure is a patently stupid move.
I experienced that at a US DOE operation where we were tasked with providing security response protecting Special Nuclear Materials.
If either/both of the two Pilots assigned to the aircraft was called for a Pee Test....the aircraft was removed from Service and the SRT guys had to revert to ground transport should a call out occur.
The drug test could not be taken AFTER you completed your duty period....it had to be done before you began a Duty period.....the rub was we were already on duty when the Notices arrived from the Admin folks.
When you develop your Safety Policy and Procedures....they must be very carefully crafted to. prevent these kinds of things from undermining what is written on paper by what happens in reality.

Some guy doing his job dies because those above him forget that there was someone going to be in the dirt on the ground checking while they ignoring the concerns that were said to them. Some careers should finish as a result of this.
