Rather than start a new thread please give us your input on the above at the

Same reason people are dying in loss of pressn incidents. AVMED is taught early and poorly (and forgotten soon). Military do regular refreshers. A hand-out by simulator staff would assist. Maybe just a good book made mandatory by the regulator - and a random pertinent question asked at licence upgrade/renewal flight test?
