Dear Doctor,
Kindly note, that we require the medical reports specified below along with the Pre-Employment medical form filled out and signed by you.
Please perform the following examinations and provide the accompanying reports (IN ENGLISH):
- Detailed Chest x-ray report with the comment on Pulmonary/Cardiac findings and Thoracic SPINE; if scoliosis noted please provide cobb’s angle report
- Complete Blood Count (CBC) with differential count and reference range
- VDRL, HAV IgM, HBs Ag, HCV Ab, HIV 1&2
- Eye test report (Only on the basis of Snellen Chart in "20/20" Visual activity format), Color vision using Ishihara method
- Detailed Audiogram report with Graph
- 12 lead ECG tracing (Graph) with report/comments on findings
Please ensure that the points 7, 8 and 15 in the attached form are filled out.
If any abnormality is noted please provide detailed reports.
